Prevail Mental Health (Opamen Practice PLLC) is required by law to maintain the privacy of your protected health information (PHI) and to provide you with this Notice. We are required to abide by the terms of this Notice currently in effect.
How We May Use or Disclose Your Health Information
Treatment: We may use or disclose your PHI to provide, coordinate, or manage your mental health treatment. Payment: We may use or disclose your PHI to obtain payment for services, including billing your insurance. Healthcare Operations: We may use your PHI for quality improvement, training, and administrative purposes.
Your Rights Regarding Your Health Information
You have the right to: inspect and copy your health information; request an amendment to your records; receive an accounting of disclosures; request restrictions on use and disclosure; request confidential communications; and receive a paper copy of this notice.
Emergency Disclaimer
Prevail Mental Health does not provide emergency or crisis services. If you are experiencing a mental health emergency, please call 911, go to your nearest emergency room, or contact the 988 Suicide & Crisis Lifeline by calling or texting 988.
Mandatory Disclosures
As required by law, we may disclose PHI without your authorization when: required by state or federal law; necessary to prevent serious and imminent harm; required to report child or elder abuse/neglect; or ordered by a court.
Questions or Complaints
